All physicians are Board Certified through the American Board of Family Medicine and the American Academy of Family Physicians. Every Nurse Practitioner and Physician Assistant is fully licensed and board certified in the State of New York. Working closely together, our entire team is committed to providing exceptional family-focused medical care.

Our Diabetes Care Management Team is nationally recognized by the National Committee for Quality Assurance and the American Diabetes Association for excellence in diabetes management and patient education.

At CNY Family Care, LLP, we prioritize ongoing, comprehensive healthcare for every patient. To support this model, we ask each patient to choose a personal Primary Care Physician. This physician will be your main point of contact, oversee your long-term care, and lead the healthcare team that includes nurses, Nurse Practitioners, Physician Assistants, and Diabetes Educators. Together, they ensure you receive timely preventive screenings, age-appropriate care, and the most up-to-date, evidence-based treatments for your health needs.

We do not accept Workers’ Compensation insurance. At this time, we are only welcoming new patients who are infants, adolescents, or young adults transitioning from pediatric care. We are currently unable to accept new adult patients.
